Pharmacists are skilled professionals delivering essential health care, but mistakes by pharmacists and other health care professionals occur with almost alarming frequency. According to The Quality of Health Care in America Project, established by the National Academies’ Institute of Medicine (IOM), errors committed within the health care system have significant impact on patient safety.
